Job Description:

This position provides assistance to the Craniofacial Center team by actively participating in contract quality improvement initiatives and assisting its patients in gaining access to needed medical, social, educational, and other services. This includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services.

Duties to include:

  • participate in weekly craniofacial clinic, weekly administrative staffing and post clinic patient reviews. They will ensure that referrals to the craniofacial clinic are processed timely with adequate follow up.

  • ensure that referred patients and families have access to community resources and support.

  • provide rapid response to emergency needs of complex patients and families by providing appropriate interventions to stabilize, improve and resolve crisis situations.

  • document assessment and treatment plans in EPIC (Electronic Medical Record) in a timely manner and implement documentation initiatives.

  • provide assistance with the development of linkages with providers and community services that will enable families to achieve the best outcomes.

  • participate in ongoing CMS contract QI initiatives, for both local and state projects. This includes monthly meetings, actions plans, learning sessions, and in-person events.

  • assist with CMS contract deliverables, quality improvement projects, needs assessment policies, transition policies, and mental/behavioral health policies.

  • provide craniofacial team with training and education on psychosocial factors, social determinants of health, transition and patient family-centered care in mezzo and macro-level settings.

  • meet with patients and families to provide assistance with school accommodations for IEPs and 504s.

  • develop school-related educational materials for patients and families.

  • screen and assess complex patient and family strengths and needs, services and financial resources that may impact the patient’s health status.

  • assist patient care coordinator with management of care for complex patients.

Normal work hours are Monday though Friday, 8:00 am to 5:00 pm; Attend annual craniofacial speech and family camp held on weekend once/year each.

Expected Salary:

$57,000-$65,000; commensurate within this range based on qualifications and experience

Minimum Requirements:

Professionals with one of the following credentials are qualified to provide psychosocial counseling:

  • Social worker with a master’s degree or a Ph.D. in social work from a Certified Social Work Education-accredited school of social work.

  • Registered nurse with advanced specialized counseling education and training as a clinical nurse specialist or certified psychiatric nurse.

  • Professional licensed to provide clinical, counseling, and psychotherapy services by the Florida Department of Health, Division of Medical Quality Assurance.

  • Counselor with a master’s degree in counseling.

  • Psychologist with a master’s or Ph.D. in psychology.

  • Master’s or doctoral level graduate student intern from an accredited school of social work or a psychology or counseling program, under the supervision of a licensed practitioner qualified to supervise such interns.

Preferred Qualifications:Master’s degree with experience in health care to provide psychosocial services to the patient and family and to assist the team in case management and coordination of needed services.
